Healthy foods cost more than high-sugar, high-calorie ones that can lead to disease , According to Statistics Canada figures from 2006, the average Canadian consumes 31.35 kilograms of sugar a year, or roughly 22 teaspoons (or 22 cubes) of sugar a day. If it's not easy for a retired university professor with diabetes to eat right, imagine how difficult it is for the poor. Rene, a single mom of two who lives on disability, spent $7 on bread and milk at a convenience store the other week because she didn't have enough money to take the TTC to the nearest grocery store. When she does get to the Price Chopper, she buys hamburger on sale, macaroni and cheese, and hot dogs. She gives the nutritious food she buys to the kids first. "That is why a lot of people on assistance are fat," she says. "They eat the fillers so the kids can eat the nutrients." She wonders how long it will be before she gets diabetes. "Am I walking that line?" she asks. "How much longer will it be before I develop it (diabetes)?" Low-income earners face an uphill battle if they want to eat healthy and fend off the ticking time bomb of diabetes, a seemingly unstoppable epidemic. The rising numbers have touched off a debate about how to get people to avoid unhealthy food. Some experts call for an all-out attack – a ban on trans fats and restrictions on sodium and glucose-fructose in processed foods. Others say we need to subsidize healthy foods for poor families. Earlier this month, a groundbreaking report showed the looming diabetes epidemic in Toronto is a story about social disadvantage and how urban sprawl contributes to the crisis. The Institute for Clinical Evaluative Sciences report found diabetes rates soared in low-income, high-visible-minority neighbourhoods where residents had less access to grocery stores. In Toronto alone, 225,000 people have diabetes, a 27-per-cent increase in the total number of cases in the past four years. What is happening here is mirrored throughout the rest of the world, where there are 246 million people living with the disease. Cases are expected to grow to 380 million by 2025, according to the International Diabetes Federation. Foods big on carbohydrates and calories and low on nutrition are less costly than whole-grain bread and vegetables. Poverty keeps most people away from nutritious meals and in a cycle of eating processed, cheap food that can lead to disease. Controlling diabetes by eating a wholesome diet is expensive. Matt Hughes is a type 2 diabetic. The 66-year-old Torontonian, a retired professor from Nova Scotia's Acadia University, is able to control his disease with a strict diet and exercise at the gym in his condo. Going to the supermarket is an exercise in frustration. He can't believe there aren't more sugar-free products on the shelves in a province with 850,000 diabetics. "I live by a Dominion, a Price Chopper and a Loblaws," says Hughes. "I've spoken to all the managers of all the stores. One would think there would be an interest in supermarkets carrying sugar-free products. Few do, and in very limited quantities." Hughes is unable to eat processed foods full of glucose-fructose or other sweeteners that end in "ose," as his partner, Roger Johnson, says. Johnson is the family cook and penny-pincher. They spend about $800 a month on food. "We buy nothing pre-packaged; we make everything," he says. They prefer to buy fresh food, vegetables, chicken and fish, and consider themselves lucky to live in a multicultural neighbourhood full of small South Asian and Chinese markets that sell exotic produce. "I find variety helps you stick to the diet," says Hughes. Not everyone has the time or the ability to spend part of the day preparing wholesome meals that don't come from a can, box or restaurant. Eating well is expensive – about $576.06 a month to feed a family of four. Every year, all of Ontario's public health units do a survey of how much it costs to purchase 66 foods from the Canada Food Guide. Basics – such as rice, meat and beans – are among the 66 in what is called the "nutritious food basket." In the past year, the price of the basket has risen by 7 per cent. Rene wasn't that surprised to see that her neighbourhood had high rates of diabetes according to the ICES study. The research found diabetes rates were highest in areas of Toronto that have more low-income residents and poor access to public transit, and in places where residents have long walks to get fresh fruit and vegetables. People living in poverty, those who can't afford to take public transit and those who regularly use a food bank face an uphill battle if they want to stave off disease by eating right, says Nick Saul, executive director of The Stop Community Food Centre. "When you have diabetes and are trying to control illness, it's very difficult to do that because of your local food insecurity," he says from his office at The Stop on Davenport Rd. "Our food bank is a long way from accommodating someone who says, `I'm a diabetic.'" The centre isn't just a food bank. It's a social experiment in how to teach people where food comes from and how to prepare nutritious meals on limited budgets, and how to foster a sense of community among those who come in the door. At The Stop, the average monthly household income is $1,049 – that is $4,700 less than the average monthly household income in Toronto. The median after-rent income of a Stop food bank user is $3.26 per day. Sadly, half of all food bank users go hungry a couple days every week, according to The Stop. The hampers food banks give out are meant to last a few days. It's rare that fresh meat or vegetables are handed out. Most items come from a box or a tin – macaroni and cheese, soups and corporate handouts of products they can't sell. Saul doesn't understand why the province mandates that public health units collect pricing information on the "healthy food basket" when that information in no way influences public policy. People on social assistance with conditions such as diabetes and hepatitis C receive small subsidies under a special diet fund, but Saul would like to see a food subsidy given to all those who receive government benefits.
